To generate the template with an (optional) explanation, simply type the explanation within the template.
For example {{NeedsUpdate|Job was completely overhauled}} would generate:
Template:NeedsUpdate/doc has had some changes, this article might not be (fully) up to date
Reason: Job was completely overhauled
